REGISTRATION REQUIREMENTS:
Every player is required to provide the following items to the AVYAA Pop Warner organization by July 13th Opening Day. Your child will not be issued equipment or uniforms, nor will they be allowed to practice or be on the field until all requirements are met.

1. 2008 National Pop Warner Player Registration Form
(players name must match birth certificate exactly)
2. 2008 National Pop Warner Proof of Physical Fitness
(dated after January 1st 2008 and signed by physician)
3. 2008 Almaden Pop Warner Registration Form- SIGNED 2 sides
4. 2008 Almaden Pop Warner Code of Conduct Form- SIGNED 2 sides
5. Copy of Birth Certificate(Xerox Copy- No Originals)
6. Copy of complete Year-End report card for 2007-2008
7. 2x3 Head shot photo
8. Copy of Insurance card
9. Volunteer Sign up or $150 Buy-Out option paid in full.
10. $375 Registration fee paid in full ( If required )+$125 equipment deposit.
*All fees must be paid in full by Opening Day JULY 13th, 2007 to secure a roster spot. We register on a first come first serve basis.
